Form Description Instructions
Acceptable Proof of Out-of-State Residency Non-Massachusetts residents looking to reinstate their right to operate in Massachusetts after two or more Operating Under the Influence (OUI) convictions are required to provide proof of residency. Please review and submit one of the documents from the list to a Hearings Officer, or Driver Control Unit, Attention Interlock Program, PO Box 55896, Boston, MA 02205. Proof of residency is required in addition to completed Ignition Interlock Operator's Affidavit for Out-of-State Residents.
Activity Hold Request Use this form to request that an activity hold be placed on your license or identification card if you believe your information has been compromised in any way. Complete and sign the form. Bring the completed form to any full service RMV office for processing.
Application for Intrastate Medical Waivers To Operate Class A,B, Or C Commercial Motor Vehicles
This form is used to request a medical waiver to operate Class A, B, or C Commercial Motor Vehicles.

A copy of the results of a recent DOT medical examination performed pursuant to 49 CFR 391.43, upon which the examining physician has indicated that you are only qualified to operate a commercial motor vehicle with an intrastate medical waiver. Your employment driving record for the previous five years. Your current employer's letter(s) of recommendation, including a certification that you will be engaged only in intrastate commerce within the Commonwealth of Massachusetts.

Application to Accept an Out-of-State Motorcycle Rider Education Certificate This form is used to convert an out-of-state motorcycle rider education certificate to Massachusetts.

Complete all required sections. Enclose a check or money order and mail to the address on the form.
Application to Convert Out-of State Driver Education Certificate
Use this form to convert your out-of-state driver education certificate to a Massachusetts driver education certificate.

Mail a copy of your out-of-state certificate, and a check for the required $15.00 fee, to the RMV Professional Driving School Section, P.O. Box 55889, Boston, MA 02205-5889.

CDL Application

 Use this form to:
  • Apply for a CDL license
  • Apply for a CDL knowledge/permit test
  • Change the information on your CDL license
  • Convert your out-of-state CDL license
  • Renew your CDL license
  • Apply for a duplicate CDL license
 Complete the General Applicant Information section and all other sections (including the Signature section) related to your transaction. All questions in the Required Information section and the Voter Registration section must be answered regardless of the type of transaction.  Bring the completed form to any full service RMV branch office with the appropriate fee.
CDL Road Test Application

 Use this form to apply for a CDL road test. Complete the General Information section, the Required Information section, the Sponsor Information section, the Vehicle Requirements section, and the Signature section.

Ignition Interlock Affidavit - Additional Licensed Resident This affidavit must be completed by any new licensed resident living in a household with a driver who is required to have an Ignition Interlock Device installed in their vehicle. Please complete this affidavit and mail to: Registry of Motor Vehicles, Driver Control Unit, Attention Interlock Program, PO Box 55896, Boston, MA 02205.
Ignition Interlock Affidavit - Other Licensed Resident This affidavit must be completed by each licensed resident living in a household with a driver who is required to have an Ignition Interlock Device installed in their vehicle. Please complete this affidavit and submit to a Hearings Officer at the Registry of Motor Vehicles with proof of installation of the Interlock Device and any other affidavits that are required.
Ignition Interlock Operator's Affidavit This affidavit must be completed by drivers who are required to have an Ignition Interlock Device installed in their vehicles. Please complete this affidavit and submit to a Hearings Officer in addition to a proof of installation of the Interlock Device and all other required affidavits.
Ignition Interlock Operator's Affidavit for Out-of-State Residents This affidavit is required by non-Massachusetts residents looking to reinstate their right to operate in Massachusetts after two or more Operating Under the Influence (OUI) convictions. Please complete this affidavit and submit with an acceptable proof of Out-of-State residency to: RMV Driver Control Unit, Attention Interlock Program, PO Box 55896, Boston, MA 02205.
Class D, M, or D/M License and ID Card Application
 Use this form to:
* Apply for a Learner’s Permit Exam
* Apply for a Class D, Class M, or Class D/M permit or license, or for a Mass ID Card or Liquor ID Card
* Renew a Class D, Class M, or Class D/M license, or a Mass ID Card or Liquor ID Card
* Change any information (i.e. name or address) on an existing Class D, Class M, or Class D/M license or permit, or Mass ID Card or Liquor ID Card
* Apply for a duplicate Class D, Class M, or Class D/M permit or license, or Mass ID Card or Liquor ID Card
* Convert an out-of-state Class D, Class M, or Class D/M permit or license

At the top of the application, indicate which transaction you are applying for. Complete the Identification Requirements section, the General Information section, and all other sections (including the Signature section) related to your transaction. All questions in the Required Information section and the Voter Registration section must be answered regardless of the type of transaction. Bring the completed form to any full service RMV branch office with the appropriate fee.

Class D or M Road Test Application
 Use this form to apply for a Class D or M road test.

Complete the General Information section, the Signature section, and the Sponsor Information section. If you are under 18 years of age, the Parental Consent For Minor section must be completed by a parent/guardian.
Request for Driving Record
 This form is used to request a driving record or history for a MA driver. Complete all required sections. Enclose a check or money order and mail to the address on the form.
Request for Personal Information Form (Individual or Multiple Records)
This application is used to request personal information from the Registry of Motor Vehicles. Please read the Personal Privacy Protection Act before downloading or using this form.
7-D School Pupil Transport License Application
 Use this form to apply for or to renew a 7-D license. Complete and sign the form in the appropriate places. Make sure your physician completes his/her section on the bottom of the form.  Mail the application and correct fee to the address on the form.
School Pupil Transport Diabetes Medical Evaluation Form This form is a supplement to the 7D-School Pupil Transport License Application and must be submitted by an applicant for a School Pupil Transport License who is currently diagnosed with diabetes and they are either insulin dependant or have had a hypoglycemic episode or spell. The form must be completed in its entirety and signed by a medical doctor that is board-certified or board-eligible in endocrinology. In addition, the applicant must agree to adhere to the conditions outlined in the form, and sign the form in the presence of the endocrinologist. The form must then be submitted in conjunction with a completed 7D-School Pupil Transport License Application.
School Bus and School Pupil Transport (7D) Operator Cardiovascular Medical Evaluation Form This form is a supplement to the RMV 7D-School Pupil Transport Certificate and the DPU Bus Driver Certificate Application and must be submitted by an applicant for a School Pupil Transport or School Bus Driver Certificate who has an implanted cardiac defibrillator (AICD).
 The form must be completed in its entirety and signed by a medical doctor. The form must then be submitted in conjunction with a completed 7-D School Pupil Transport Certificate Application.
Vision Screening Certificate This form is used in lieu of the RMV eye exam and must be signed by an optometrist or physician. Have your eye care provider complete and sign this form in the appropriate sections.  Bring the completed Vision Screening Certificate with you when you come in to apply for or to renew your license.
