Activity Hold Request Use this form to request that an activity hold be placed on your license or identification card if you believe your information has been compromised in any way. Complete and sign the form. Bring the completed form to any full service RMV office for processing.

Driving Complaint Form

 This form is used to file a complaint with the RMV pertaining to the improper operation of a motor vehicle. Complete the form, including all pertinent information. You may attach extra sheets as needed to complete the description of improper operation. The form must be signed in order to initiate an investigation. Mail the completed form to the address on the form or hand deliver it to any RMV branch office.
Elder Outreach Form
(revised 4/07) 		This form is used by senior centers and Councils on Aging to request one of the Registry's outreaches for elder drivers. Please complete the required information on the form and return it by fax or mail. The fax number and address are found on the form.
High School Outreach Form
