Application for Antique Motor Vehicle and Year of Manufacture Plates

Use this application to request permission from the RMV to use an acceptable antique or year of manufacture plate.

Once you have received permission to use a Year of Manufacture Plate, an Antique Normal Plate or you have received the postcard informing you that your Antique Vanity Plate is in the requested RMV branch office, you must present this completed Application with the appropriate documentation and applicable fees to a full service RMV branch office to register your vehicle. You must have this Application signed and notarized before registering your vehicle.

Application for Gold Star Family Plates

This application is used to apply for Gold Star Family plates. A Gold Star Family plate can be issued without charge to spouses, parents, and children of soldiers who have been killed in action while serving in the Armed Forces of the United States.

Please complete this application and submit it, along with the required documents, to the nearest full service RMV branch office.

Application for Ham Radio Operator Plate

This form is used to apply for a Ham Radio Operator plate.

Complete the Ham Radio Operator application. Mail the completed form, a copy of your FCC license, and a $20 check or money order, payable to the RMV, to the address listed at the top of the application.

Application for International Registration Plan (IRP)

Use this application to enroll in the International Registration Plan (IRP).IRP is an optional program for licensing commercial vehicles traveling in two or more IRP jurisdictions. A jurisdiction may be a state or a Canadian province.

Application for Lottery Plate Re-assignment

Use this form if you currently have a Lottery plate and you want to re-assign the Lottery plate to an immediate family member or to someone with whom you have a significant personal relationship.

You must have the Application for Lottery Plate Re-Assignment notarized before submitting it to a full-service RMV branch. If re-assigning the plate to someone with whom you have a significant personal relationship, you must also complete a Lottery Plate Re-assignment Affidavit.
Application for Medical Doctor(MD) Plate

This form is used to apply for a Medical Doctor plate. Only physicians who are actively registered by the Board of Registration in Medicine are eligible for these plates.

Complete the Medical Doctor application. The Board of Registration section must be completed by an official at the Board of Registration in Medicine. Bring the completed application to any full service RMV branch. Pay the appropriate fees to obtain these plates

Application for Veteran Plates

This application is used by qualified veterans to apply for veteran's plates for their passenger vehicles or motorcycles. *Specific Medals may be found on Application for Awarded Medal Veteran Plates.

Application for Motorized Bicycle (Moped) Registration This form is used to register a motorized bicycle (also known as a moped). This form is screen-fillable, which means you may print and complete it by hand or complete it on screen and print it.

Application for National Guard Plate

This form is used to apply for a National Guard plate. Only active members of the National Guard who have an E2 rating or higher, or members who have retired after 20 or more years of service, are eligible for National Guard plates.

Complete the National Guard application. The Certificate of Massachusetts National Guard section must be completed by an official at the Massachusetts National Guard Headquarters. Bring the completed application to any full service RMV branch . Pay the appropriate fees to obtain these plates.

Application for News Photographer Plate

This form is used to apply for a News Photographer plate. Only active members of the Boston Press Photographer’s Association are eligible to apply.

Complete the News Photographer application. The application must be certified by the Boston Press Photographer's Association. Mail the completed form to the address listed at the top of the application

Application for Registration Hold by a Member of the Military

This application can be completed by any Massachusetts resident who is being deployed to, or is present in, a theater of combat operations.  If you qualify, you are permitted to retain one or more sets of registration plates for the duration of your deployment. These plates will be cancelled and put in a "hold" status. They will be reactivated for no fee when you return.

Complete the application.  Bring the completed application, along with photocopies of your deployment papers, to any full-service RMV branch, or mail them to the address on the application.

Application for Vanity Plates

This application is used to apply for a vanity plate.

Complete the application and be sure all owners sign it. Bring the completed form to any full service RMV branch office. Once an RMV representative has approved your application, you will be required to pay the appropriate fee to have your plate ordered for manufacture.

Application for Awarded Medal Veteran Plates (revised 11/07)

This application is used to apply for Silver Star, Bronze Star, Congressional Medal of Honor, Legion of Valor, Pearl Harbor Survivor, and Distinguished Flying Cross plates.

Approval Process for Electronic Insurance Stamps and RMV Forms

Massachusetts has a process for insurance companies to produce an electronic signature and insurance certification stamp for their customers’ registration applications. These are the guidelines to help with the approval process.

The RMV must approve all electronically generated forms and insurance stamps prior to implementation. Contact information and instructions are included in this document.

I-Pay-Tax Complaint Form

This form is used to report Massachusetts residents who are evading taxes by improperly registering their vehicle in another state. This form must be filled out completely and accurately for the violation to be investigated. All information will be kept strictly confidential.

IRP Affidavit for Cancellation of a Registration

This affidavit must be completed by any customer canceling an IRP Registration. Complete and return this affidavit, along with the CAB CARD and IRP plates (apportioned plates) to any RMV branch location.
Lost Plate Affidavit form
(revised 04/07)
This form is used to cancel a Registration without the plates.

Mail the completed C-19 form to the address below. Your C-19 form will be mailed back to you with a Registrar's stamp on it. Make a photocopy for your insurance company and keep the original for your own records. You need to provide the Assessor's Office from your town with a copy of your plate cancellation receipt and documentation on the whereabouts of your vehicle so they can process any excise tax adjustment that may be due.

Nonresident Student Vehicle Information Form This form applies to students commuting from out-of-state to a school in Massachusetts and students temporarily residing in Massachusetts. This form is screen-fillable, which means you may print and complete it by hand or complete it on screen and print it. This completed form must be filed with the Police department in the city or town where the school is located. The Police department must provide a copy for the school and the RMV. The school will issue the required decal, which must be affixed to the uppermost center portion of the windshield. The penalty for failing to comply as required is up to $50.00.

  • Summer/Winter Swap (SW)
  • Title Add Registration (TAR)
  • Surviving Spouse (SS)
This is a 2-page document.  Both pages must be completed in full, stamped by your insurance company or agent (registration transactions only) and submitted with all proper documents to the RMV branch office for processing.  Purchaser(s) signature is required on line 50 / 51.  Mass dealer signature required on all Mass dealer sales.

This form will be rejected by the RMV for the following reasons:

  • The form is not printed on 8 ½ x 11 white paper
  • The form is printed on color paper
  • The form is printed on card stock
  • The form is not signed in the appropriate places by the registered owner(s)
  • The form is not legible due to ink smears or low toner marks
RMV-3 Form

Use this form to change your insurance company, to reinstate your insurance, swap to a different plate, amend information on your registration, or to renew your registration.

Do not use the RMV-3 Form if there are any changes in ownership or you are requesting a summer/winter swap. In these cases, an original application for title (RMV-1 form) must be completed.
