Excise Correction Form City and Town Assessors and Tax Collectors may use this form to make corrections for garage codes, addresses and other excise related matters. Complete the Excise Tax Correction form with the required changes. The form may be mailed to the following address: Excise Tax Corrections, Registry of Motor Vehicles, PO Box 55889, Boston, MA 02205-5889. Or they may be faxed to 617-351-9404.
Tax Collector's Notice of Excise Tax Delinquency by General Registration Holder This form is used by city and town tax collectors to report delinquent excise tax by general registration holders. Complete the Tax Collector's Notice of Excise Tax Delinquency by General Registration Holder and mail it along with a letter on official stationery. The letter must reference "Notice of Excise Tax Delinquency."
