Option 4: Refund/Rollovers - contact if you plan on leaving state service and would like to request a refund of your contributions.
Option 5: Survivor Benefits - contact with beneficiary questions, to notify the Board of the death of a member.
Option 6: Disability Retirement Counseling - contact if you are suffering from an injury or illness which prevents you from doing the duties of your position.
