Benefit Recipients who did not respond to our initial Benefit Verification Form mailing were mailed a second notice on Thursday, July 17th. The requested return date has been extended to Friday, August 15, 2008. If you already returned your BVF, thank you. If you would like to confirm that we received your form, please contact us.
