| SPECIAL NOTICE FOR SMART PLAN PARTICIPANTS
In an effort to ensure you have the most up-to-date records on file, please take the time to review your beneficiary information with the SMART Plan. Your beneficiary designation filed with ING remains effective with the new service provider, Great-West Retirement Services. However, it is important that you verify the beneficiary designation is correct or updated to ensure your assets will pass on as you intended.
Updating your beneficiary is quick and easy. You have two choices:
Online- Click Account Access. After logging in, go to My Profile > Update Beneficiary.
Paper- Click Find A Form. Click on the appropriate Beneficiary Designation form. Mail or fax the completed form to the address or fax number provided on the form. You will receive a written confirmation after your beneficiary information has been updated.
If you have any questions, please call the SMART Plan Service Center at 1-877-457-1900 and press 0 for an operator. Customer Service Representatives are available to assist you Monday through Friday from 9:00 AM to 8:00 PM ET. |
