Effective May 1, 2019, any Itemization of Services submitted that doesn't comply with the following guidelines will be returned to the vendor for corrections, prior to submission to the Office of Court Management for payment.

Important information re: Guardian and Guardian ad litem Itemization of Services

Please review these Itemization of Services Instructions.

  1. Dates of Service – List the month, day and year on which the service was performed.

  2. Start Time/End Time – List the time you started and the time you finished the service performed.

  3. Time Spent – List the amount of time spent in minutes you spent on the service performed

  4. Description of Work – Describe and specify the work/service performed for this on the Date of Service. The description should include the type of service (meeting, telephone calls, court appearance, report writing, etc.), the purpose and content of the activity, the participants of the activity (meeting participants, telephone call participants, etc.), and the outcome of the activity. Kindly be as explicit as possible.

  5. Total Minutes – Total the aggregate minutes worked in the TIME SPENT column for all dates of service.

  6. Total Amount Billed – Divide the total minutes by 60 to determine hours worked. Multiply the hours by the corresponding hourly rate to determine the total amount. For multiple Itemization of Services pages on the same case, add the Total Amount Billed from each page to determine the aggregate total invoice amount.
