Request for Verification of Massachusetts Residency Did you recently get a letter asking for proof that you live in Massachusetts? You got the letter because MassHealth has information that you (or someone in your household) have an open Medicaid case in a state other than Massachusetts. The other state is listed on the letter you got. In order to keep getting MassHealth benefits, you must send in proof that you live in Massachusetts. We must get this proof within 45 days of the date on that letter. Please use the envelope we gave you and include a member ID # with your proof. Examples of acceptable types of proof are: • lease • city or town real estate tax bill • notarized rent receipt • current utility bill • notarized affidavit supporting residency If the letter was for a child, please send in proof that your child went to a Massachusetts school for this school year. If you have been on Medicaid in the listed state (or another state), or if you think someone is using your social security number, please call the Fraud Hotline at 1-877-437-2830 for help.
