FY25 DER Culvert Replacement Grant Application

This form is for submitting applications to DER’s Culvert Replacement Municipal Assistance (CRMA) grant program and Training Site Initiative

Instructions: 

  1. Review the RFR document, which includes detailed application instructions and definitions for terms in this form (Section 3), as well as necessary information about the grant’s purpose, eligibility requirements, and scoring criteria.
  2. Gather required and optional documents for upload, including:
    • Completed Budget Template (required)
    • Project Photos (at least two required)
    • Supporting Documents (as applicable) – including but not limited to field data notes and/or summary technical reports, design plan sheets, cost estimates, design or construction bids, etc.
    • Documentation of Owner Support (as applicable)
    • Please note: The application system has a strict 25 MB limit total for all photos and documents uploaded per-applicant (not per-file). Applicants should reduce file sizes and/or be selective of which photos and documents to upload as needed to meet this limitation.
  3. Complete the application form below, including uploading any documents. Applications must be received by 4:00 p.m., Friday, April 12, 2024. Any partial application or application received after the deadline will be rejected. All applications must be submitted through this portal. No paper applications will be accepted. There is no login ID or password required.
    • The application can be saved while working on it by clicking “Save and Resume Later” at the bottom of the application and then clicking on “Save and Get Link.” A link to your application will be provided which must be copied and saved in order to return to the application. A good way to ensure that the link is not lost is to email it to yourself. Please note: any documents or photos that have been uploaded will not be saved if leaving the site before submitting the application. Documents and photos will need to be re-uploaded again upon re-entering the site and finishing the application. Failure to enter information in any of the required fields or to submit required attachments may result in the disqualification of the application. 

Please note if there is an error in this form, the uploaded materials will not be saved and you will need to reattach all of these documents before you submit.
