What you need

Needed to fill out the application

  • Your Documentation of DPH / RCP licensure
  • Your DPH / RCP license number
  • Pharmacy ownership information
    • If it's owned by an individual, state the:
      • Name of owner, address, and their Social Security Number
    • If it's owned by a partnership, state the:
      • Partnership name, address, and FID number
    • If it's owned by a corporation, state the:
      • Corporation name, address, and FID number
      • Company in which it's incorporated
      • Names of the Corporate Officers
  • Information on the pharmacist charged with the management of the pharmacy
    • They must be a Massachusetts registered pharmacist, and have been qualified by the Board as a nuclear pharmacist
    • You need their:
      • Name
      • Mass. Pharmacist Registration Number
      • Date of qualification by the Board as a nuclear pharmacist
      • Social Security Number
  • Name of other Massachusetts registered pharmacists in employment
    • If they are registered as nuclear pharmacists, state their regulation numbers


  • Pay by check or money order, payable to the Commonwealth of Massachusetts
  • Fees are non-refundable
