What you need

  • You will need to have either Adobe Acrobat or Adobe Reader installed on your computer to complete the application forms
  • Before completing the application forms, save the forms to your computer and complete them off-line using Adobe Acrobat or Adobe Reader. Application forms should be saved often during completion to avoid loss of changes
  • All responses must be legible within the response fields
  • All signatures within the application forms must be wet signatures. Electronic signatures will not be accepted.
  • Please keep a copy of all application materials you submit for your own records

Please complete all application forms and supplementary documents before applying. We are unable to process incomplete applications.

Applications are not scored, but instead reviewed to determine if they comply with Chapter 369 of the Acts of 2012, An Act for the Humanitarian Medical Use of Marijuana (the “Act”), and DPH regulations at 105 CMR 725.000 (“Regulations”).
  
Background checks will be conducted by an outside vendor during the application process in compliance with the guidance for RMDs regarding background checks.

If an individual associated with a proposed RMD is deemed unsuitable, the RMD may elect to remove that individual from the application and submit another individual for the proposed role during the application process.
  
Throughout the application process, and after receiving a Provisional or Final Certificate of Registration, applicants and registrants will be responsible for remaining in compliance with guidance for RMDs regarding non-profit compliance if applying as and operating as a non-profit, and guidance for RMDs regarding background checks.

In order for an RMD with a Provisional or Final Certificate of Registration or an applicant with an application pending before the Department to convert from a non-profit corporation to a domestic business corporation, they must follow the procedure outlined in the RMD corporate conversion guidance.
