Each municipality and non-municipal public agency needs a single Massachusetts Controlled Substances Registration (MCSR) for their public employees to administer naloxone or other approved opioid antagonists.
Processing may take between 2 and 3 business weeks from the receipt of application.

What you need for Apply for MCSR - Naloxone

  • Municipality or public agency name
  • Applicant address
    • We can't process applications with a P.O. box number and no street address
  • Applicant telephone number
  • Applicant Federal Tax ID Number (Required by M.G.L. c. 30A, s. 13A)
  • Name of Medical Director (authorized licensed physician)
  • Attach copies of the Medical Director’s Board of Registration in Medicine License and MCSR.
    • Only send copies of supporting documents. Don't send originals, as we won't return them.
