Reimbursement
If we approve your application, you will get:

A letter showing your approved award amount

A W9 form

A satisfaction survey to fill out after the event
Within 30 days after the event, please send us:

A written request for reimbursement

Any accompanying paid receipts from the event

A completed W9 form

A completed satisfaction survey
Follow-Up Survey
About 6 months after you receive the funds, we will send you a voluntary follow-up survey. This survey helps us see the impact our support has on you in achieving greater independence, productivity, and inclusion. Though the survey is voluntary, you must complete it to be able to get funding in the future.

Only Massachusetts residents can receive funds.
Council approval of funding for any event is not an endorsement of the event or of organizations sponsoring or involved in the event.
The Council will provide a maximum of $750.00 per event regardless of how many people apply for that event. If more than 3 eligible applications are approved for the same event, the amount of funds awarded for each individual approved application will be reduced (less than the maximum $250).
Limitations

Standard Mileage Rate: $0.585/mile
-
Standard Meal Rate: $30.00/day (only reimbursable for multi-day events requiring an overnight stay)

Education Materials: There is no reimbursement for event materials, including, but not limited to, books and videos.
