What you need   for Enroll in the Board of Registration in Nursing, Substance Addiction Recovery Program (SARP)

Before completing an application, please review the Eligibility Criteria.

Nurses may enter the program while being prescribed a medication to treat substance use disorders such as Suboxone or Methadone. Please review the policy for entry into SARP while being prescribed either medication by viewing the policy SARP 13-01 SARP Eligibility Criteria.

Admission process

Please be advised that all required documents are expected to be submitted in one submission. Entry into SARP generally follows the process below: 

Step 1: submit all required documents

To obtain the SARP application bundle, please call the confidential SARP line at (617) 973-0904 and include the following:                                        

  • Your name
  • The mailing address the application is to be sent to
  • Your phone number

SARP staff will mail the application to you.

SARP staff may contact you with questions.

How to submit   Enroll in the Board of Registration in Nursing, Substance Addiction Recovery Program (SARP)

Applications and all other required documents must be sent in one submission, by mail, to:

Partial submissions will not be processed. It is recommended that you send these documents by Certified Mail to ensure tracking. Please keep a copy of your documents for your records.

  1. SARP staff review

    • SARP staff will review the documents after all of the documents are received to ensure the application is complete,  
    • SARP staff reviews the application to ensure compliance with all current policies,  
    • SARP staff will communicate with the applicant about matters relative to the application and admission,  
    • SARP staff will work to draft and send the consent agreement to the applicant. 
  2. Consent agreement & urine sample

    • Applicant reviews the consent agreement and completes the SARP orientation with SARP staff,  
    • Applicant will contact to register and arrange to provide a urine sample prior to admission by the Board’s drug testing management company. This urine must have negative results for substances with potential misuse/addiction.  Refer to www.drugabuse.gov/drugs-abuse to review substances that have potential for  misuse.   3. Final enrollment

    • The applicant signs and sends the consent agreement to SARP staff,  
    • The consent agreement is effective when countersigned by Board of Nursing staff. 

    **Note: There may be instances where the applicant for admission is reviewed and approved by the Substance Addiction Recovery Evaluation Committee (SAREC) and the Board of Registration in Nursing
