What you need for Petition for license reactivation for pharmacists and technicians
- Documentation of completion of required continuing education (CE) units during the 2 calendar years before the petition. Pharmacy technicians don't need to provide CE documentation.
- 20 CEs for each calendar year of a 2 year renewal cycle
- CE must be approved by either:
- The Board
- The Accreditation Council for Pharmacy Education (ACPE)
- The American Medical Association (CME Category I)
- Other State Boards of Pharmacy
- If required CE documentation is not provided, you must complete 3 additional CEs must for each deficient credit
- A written statement from each employer since the expiration date of the license. Include dates of employment and job functions.
- Certified statement from each state of pharmacist licensure (current, expired, and inactive) verifying license status and any disciplinary action regarding the license since the date of issuance of each license
- Payment of all past due license fees and a reactivation fee. For more information on the reactivation fee, please contact the Board.

Re-training and Re-examination
If you have not engaged in the practice of pharmacy in any jurisdiction since the date of license expiration, we may require a personal appearance to review the petition and determine appropriate conditions to resume the practice of pharmacy. Depending upon the petition, the Board may determine that re-training and re-examination reactivation conditions are necessary, including but not limited to:
- An internship/experiential period
- The North American Pharmacist Licensure Examination (NAPLEX)
- A clinical educational program
Multistate Pharmacy Jurisprudence Examination
Following Board approval of a license reactivation petition, the individual must pass (75%) the Multistate Pharmacy Jurisprudence Examination (MPJE). MPJE registration and study guide information is available through the Board’s application vendor Professional Credential Services, Inc. (PCS).
