  • Describe the purpose for the renovation/expansion
  • Describe all the changes that will take place to the pharmacy in this renovation/expansion (attach more sheets if needed)
  • If the square footage of the pharmacy will change with the renovation, please explain
  • List projected dates for the renovation/expansion
  • Submit a certified blueprint or certified floor plan reflecting the renovations/expansion
  • You must also comply with all state/local building codes as they relate to this facility
  • Adequate measures must be in place to maintain the security of all controlled substances at all times during the renovation/expansion
