Police departments desiring to utilize the MPTC Medical Advisor's services must execute a Memorandum of Agreement (MOA).
The MOA expires after a period of one year.

Departments must log into the MPTC Acadis Portal to complete the MOA via a webform. The webform collects information related to your department needs for medical advisement.

How to request   Request a Memorandum of Agreement (MOA) for MPTC Medical Advisor

To start, please log into your MPTC Acadis Portal account and follow these instructions:

  1. Click on "Resources"
  2. Click on "MPTC Medical Advisor MOA" webform and complete
  3. Once you have completed the webform, MPTC will review your submission and issue a MOA Certificate that will be visible in this Portal. To find your MOA and Certificate, click on "Organization" in this Portal.

Your department's MOA expires after a period of 1 year. Once it is due to expire you may renew it via this Portal.
