  • $95 non-refundable and non-transferable fee (There will be a $23 handling charge for returned checks.)
  • Completed Massachusetts licensure application, including school signature/stamp, and notarized signature of the applicant
  • 2”x 2” passport-sized photograph
  • Preceptor information, if available
