- What are Community Partners (CPs)?

It also discusses the two types of CPs: Behavioral Health Community Partners (BH CPs) and Long Term Services and Supports Community Partners (LTSS CPs); how they help members navigate and coordinate care; how Accountable Care Organizations (ACOs), Managed Care Organizations (MCOs) and CPs work together; implications of the CP program for providers; and contains a list of CP organizations and the service areas they cover.
