To sign up for direct deposit, print the EFT/Direct Deposit application below and once you have completed the application, sign it and mail it to the attention of Provider Enrollment and Credentialing at MassHealth. Please note that EFT requires an original signature and that is the reason for the request to be mailed back. Faxes, for example, are not accepted.
