For Consumers and Family Members

If you (or someone in your family) is experiencing visual difficulties, you must first contact your primary care physician or an eye care provider.
  • Assuming that you have met both of MCB's criteria for services (1. legal blindness and 2. residence in MA); your eye care provider will submit the result of the eye examination to the MCB Registration Department.

  • The Report will be reviewed and eligibility will be determined.

  • A Letter of Determination of Eligibility will be sent to both the patient and his/her eye care provider.

  • Once the patient is registered with MCB, a member of the MCB staff will contact the registrant soon thereafter for a complete explanation of available services that are offered by MCB.

  • All records and information are confidential and protected by the law.
