Registered Marijuana Dispensary (RMD) background checks

Background check instructions and forms for RMD applicants invited to submit a Management and Operations Profile.

The Cannabis Control Commission (CCC) will complete and review background checks on individuals associated with an RMD as part of the RMD application for a Certificate of Registration.

If an individual associated with a proposed RMD is deemed unsuitable, you may elect to remove that individual from the application and submit another individual for the proposed role during the application process.

Background check forms

Background checks for the Registered Marijuana Dispensary application process are conducted by the Massachusetts Cannabis Control Commission (CCC) contracted vendor, Creative Services Inc. (CSI).

The cover sheet, authorization forms (3 forms per individual), and payment must be submitted in a sealed envelope with the name of the Corporation and marked: authorization forms. The required forms are below:
