On January 9, 2018, the HPC made up to $10,000,000 available for the SHIFT-Care Challenge. The SHIFT-Care Challenge (SHIFT-Care) represents a significant investment on behalf of the Commonwealth in sustainable, transformative care models that seek to reduce avoidable acute care utilization. Through SHIFT-Care, grantees will receive funds to invest in promising innovations that address health-related social needs and increase access to timely behavioral health services for residents of Massachusetts with the goal of decreasing the use of costly and avoidable hospital care.
Eligible entities applied for up to $750,000 to prepare and implement an initiative according to one of two tracks during a 21-month period of performance:
- Track 1 – Addressing health-related social needs
- Track 2 – Increasing access to behavioral health care
