Product description

The Massachusetts Immunization Information System (MIIS), a web-based system that keeps track of all immunizations that healthcare providers report having administered to children and adults in Massachusetts. The system was established in 2011 and is operated by the Massachusetts Department of Public Health (MDPH) according to state law (M.G.L c. 111, Section 24M; 105 CMR 222). Beginning Tuesday, December 7, 2021 Massachusetts residents or individuals who have received a vaccination in Massachusetts may access the MIIS Patient Portal – My Vax Records to access their MIIS immunization record at https://myvaxrecords.mass.gov.

Through the MIIS Patient Access Portal – My Vax Records users will be able to:

  1. Access their MIIS records if a record is available
  2. Print or download their full immunization history certificate.

Eligibility

You are eligible to submit a request to access an MIIS record:

  1. You have received at least one vaccine in the Commonwealth of Massachusetts.

Verification

The Massachusetts Department of Public Health will review your request and attempt to match the information provided to an existing record in the MIIS.

By submitting a request, you consent to the electronic storage and transmission of the requested information, including the requested vaccine record, via the MIIS Patient Access Portal – My Vax Records and acknowledge under all applicable legal penalties that the information you have provided is true and that you have the legal authority to access the MIIS record and the information in it. You understand that you may face legal penalties for intentionally attempting to access a vaccination record that you do not have authority to access.

Absence or Inaccuracy of Vaccination Record in MIIS. The MIIS was established in 2011 and contains only those immunization records that providers have reported. Therefore, the MIIS may not contain all immunizations for every person in the Commonwealth. Contact your healthcare provider to request that any missing vaccinations be sent to the MIIS.

If you are not able to locate your MIIS vaccination record in the MIIS Patient Access Portal – My Vax Records or there are inaccuracies on the record, an Electronic Immunization Record Amendment form may be submitted along with sufficient proof of your identity to MDPH. The Massachusetts Department of Public Health will then make reasonable efforts to identify the vaccination record within MIIS if there is one, or correct inaccuracies in an existing record. Or you may contact the health care provider that administered the vaccination and request the information is sent to the MIIS or updated as necessary.

SMS service

When you opt-in to the MIIS Patient Access Portal – My Vax Records service, DPH will send you an SMS message to confirm your signup.

You can cancel the SMS service at any time. Just text “STOP” to [number]. After you send the SMS message “STOP” to us, DPH will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and DPH will start sending SMS messages to you again.

DPH is able to deliver messages to the following mobile phone carriers: Major carries: AT&T, Version Wireless, Sprint, T-Mobile, MetroPCS, U.S. Cellular, Alltel, Boost Mobile, Nextel, and Virgin Mobile.

Minor carriers: Alaska Communications Systems (ACS), Appalachian Wireless (EKN), Bluegrass Cellular, Cellular One of East Central IL (ECIT), Cellular One of Northeast Pennsylvania, Cincinnati Bell Wireless, Cricket, Coral Wireless (Mobi PCS), COX, Cross, Element Mobile (Flat Wireless), Epic Touch (Elkhart Telephone), GCI, Golden State, Hawkeye (Chat Mobility), Hawkeye (NW Missouri), Illinois Valley Cellular, Inland Cellular, iWireless (Iowa Wireless), Keystone Wireless (Immix Wireless/PC Man), Mosaic (Consolidated or CTC Telecom), Nex-Tech Wireless, NTelos, Panhandle Communications, Pioneer, Plateau (Texas RSA 3 Ltd), Revol, RINA, Simmetry (TMP Corporation), Thumb Cellular, Union Wireless, United Wireless, Viaero Wireless, and West Central (WCC or 5 Star Wireless). ***Carriers are not liable for delayed or undelivered messages***

As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive 1 message per request. General rules

Internet Conduct and Limitations of Liability. Any attempt by a MIIS Patient Access Portal – My Vax Records user, participant or any other individual to deliberately damage the MIIS Patient Access Portal – My Vax Records or undermine the legitimate operation of this service, is a violation of criminal and civil laws. A person engaging in such conduct may be disqualified from use of the MIIS Patient Access Portal – My Vax Records service without notice and may face criminal prosecution or civil liability as permitted by law.

All materials submitted by registrants in connection with the MIIS Patient Access Portal – My Vax Records become the property of the Commonwealth of Massachusetts and will not be returned.

If any discrepancy exists between these Official Terms and Conditions and other statements or materials concerning the MIIS Patient Access Portal – My Vax Records, these Official Terms and Conditions shall prevail.

These Official Terms and Conditions are subject to review and may be amended or revised at any time at the sole discretion of the Commonwealth of Massachusetts.
