Eligibility and Application for Services


Who is eligible for services?

Any resident of Massachusetts, regardless of age, who has a documented externally caused traumatic brain injury may be eligible for BISCS services. Individuals must have associated impairments in behavioral, cognitive and/or physical functioning and must be able to participate in community-based services. After an application is received, BISCS staff will determine eligibility through a review of all available medical and non-medical information related to the individual's injury.

How do I apply?

Anyone may request an application by calling (617) 204-3852 or (800) 223-2559 or by sending an e-mail to SHIPU@MRC.state.ma.us. The application is several pages long; you may want to ask family, friends or BISCS staff for assistance. After the application is received, BISCS will collect medical records to determine your eligibility for SHIP services. This process may take less time if you furnish copies of records pertaining to your initial injury.
