it is an In-state License or Certificate of Compliance, OR to
YOUR LOCAL BOARD, if it is a retail license.

- License/Permit
APPLICATIONS
- All Licenses
and Permits are to be renewed ANNUALLY. Except for the "Agent,
Broker, or Solicitor" application, new applications and renewals
may use the same form. If you need to know the renewal date of your
license, you may contact the
Agency.
- Agent,
Broker or Solicitor
(Note: All employees of the broker who solicit orders for alcoholic
beverages must be licensed under s.19A, therefore, need to fill out
the Salesman Application.)
- Agent,
Broker or Solicitor Renewal
- Airline
License A corporate airline operating within the Commonwealth
to sell alcoholic beverages for consumption on the aircraft.
- Airline
Transportation A permit to transport alcoholic beverages in its
aircraft, NOT for consumption on the aircraft.
- Bonded
Warehouse
- Certificate
of Compliance Suppliers from outside of Massachusetts to sell
products to wholesalers within Massachusetts.
- Farmer-Brewery
(Ch. 138, s. 19C) Each additional warehouse must be licensed and
therefore must fill out Application
for Storage Permit
- Farmer-Winery
(Ch. 138, s. 19D)Each additional warehouse must be licensed and
therefore must fill out Application
for Storage Permit
- Winery
Shipment(Ch. 138, s. 19F) License to ship wine only directly
to Massachusetts consumers.
- General
Express or Caterer Transportation Permit A catering business to
receive alcoholic beverages on behalfof, and transport and deliver
such to any of such operator's customers if in the ordinary course
of such operator's business. Each vehicle used for transportation
and delivery needs to be permitted by the Commission. Separate applications
must be submitted for each additional vehicle.

must be licensed and therefore must fill out Application
for Storage Permit

- Public
Warehouseman Application, Public
Warehouse Law
- Railroad
/Car (Ch. 138, s. 13) A railroad or car corporation operating
any line of railroad or furnishing refreshments within the Commonwealth,
may sell alcoholic beverages to be drunk in such cars, if the Commission
sees fit to issue a license.
- Salesman
Application For a Saleman's permit to solicit orders for alcoholic
beverages on behalf of a Wholesaler. This form includes the Application
to Transport and Deliver certain Alcoholic Beverages.
- Ship
License Application A license to sell alcoholic beverages to
passengers of any vessel or shipping operating out of any port of
the commonwealth. (Ch. 138, s. 13)
- Ship
Chandler Application For retail sales by ship chandlers of
all alcoholic beverages not to be drunk on the premises, provided
such sales shall not be for purposes other than provisioning a vessel
or shipping company using any port of the Commonwealth. (Ch. 138,
s. 13)
- Transportion
An application for a Licensee's permit to transport and deliver certain
alcoholic beverages.
- Wholesaler's/Importer's
(Ch.138, s.18) Each additional warehouse must be licensed and therefore
must fill out Application
