Transmittal Form and Number for Permit Application and Payment

Please follow these instructions for a complete transmittal form:

Step 1

Note: You must have a different transmittal number for each permit application. Click the "Get Transmittal Number" button again for a new number.

Step 2

Write down your unique, seven digit transmittal number.

Step 3
