Write down your unique, seven digit transmittal number:

Note: This number is generated by a counter and is a unique identifier for transmitting and processing your permit application. For each permit application, a separate transmittal form with a unique transmittal number must be used. If you are submitting multiple permit applications, please click the REFRESH button in your browser and the counter will provide you with another transmittal number.
