Please follow these instructions for a complete transmittal form:

Step 1

In the box below, click the button "Get Transmittal Number."

Note: You must have a different transmittal number for each permit application. Click the "Get Transmittal Number" button again for a new number.

Step 2

Write down your unique, seven digit transmittal number.

Step 3

Download the Transmittal Form: MS Word 136 KB or PDF 96 KB

Step 4

 Enter your number in the upper right corner as shown:
