  • Signed and notarized Seller Transfer of Ownership Notification
  • Signed and notarized Buyer Transfer of Ownership Notification
  • Application for Certification (Certification is non-transferable and must be newly certified with a change in ownership)

Signed and notarized forms Transfer of Ownership Notification for both the Seller and Buyer should be mailed along with the Assisted Living Certification Application  and the Application fee to:

  1. Return Assisted Living Certificates to Elder Affairs

    The current Sponsor must return all Assisted Living Certificates to Elder Affairs by registered mail within 5 days after the transfer of ownership is completed

  2. Transfer of ownership completion notification

    The new Sponsor must submit a signed and notarized statement that the transfer of ownership has been completed within 5 days after the date of closing.
