Community Health Equity Survey 2023 Mini-grantees

DPH is providing flexible mini-grants for community-based organizations to conduct outreach for the Community Health Equity Survey. Grant recipients can use the funds to support community members, particularly those who experience health inequities and are traditionally left out of public health data, in completing the survey.
