MassHealth Eligibility Record Request

An eligibility record is the summary of information MassHealth uses to decide if a person meets the rules to get MassHealth.

You or an authorized representative* (someone you trust) may ask for a copy of your eligibility records at any time. The records may include copies of notices, applications, and other documents. 

How to ask for your records after you file for an appeal

If you want MassHealth to review a decision they made, you can file for an appeal and ask for a copy of your eligibility records.

To ask for a copy of your record, follow the steps below:

1. Fill out the Fair Hearing Request Form. You can find this form by going to MassHealth Member Forms.
2. After you send the form, a worker from MassHealth will call you about your appeal and your records. If you have an authorized representative, they will call that person. Let the worker know which records you want.
3. MassHealth will send your records to you before the appeal.

How to ask for your records for any reason other than an appeal

Call the MassHealth Enrollment Center at [phone], TDD/TTY: 711. Tell them which records you want and when you want them.

The Enrollment Center worker will send you a copy of your record.

You may also visit a MassHealth Enrollment Center (MEC) and meet with an eligibility worker. A MEC is a physical location with workers who can help members with their eligibility needs. MEC locations can be found here: https://www.mass.gov/info-details/masshealth-enrollment-centers-mecs.

You can also schedule an appointment for a worker to call you. To do this, use the MassHealth appointment scheduling system at https://masshealth.hylandcloud.com/203InnovationPortal/home.

* An authorized representative is someone you choose to act for you, like a family member or other trusted person. To make someone an authorized representative, you must fill out the Authorized Representative Designation Form.
