POLST in Massachusetts

Massachusetts is transitioning from the MOLST (Medical Orders for Life-Sustaining Treatment) form to the National POLST model in Spring 2027.

MOLST to POLST Transition

Massachusetts is transitioning from the current paper-based MOLST (Medical Orders for Life-Sustaining Treatment) form to the National POLST model in Spring 2027. This transition includes the implementation of a secure, statewide electronic POLST registry (ePOLST Registry) for creating, signing, and retrieving POLST medical orders. ​

Upon statewide implementation of the ePOLST Registry, all portable medical orders for life-sustaining treatment must be completed using the electronic POLST form in the ePOLST Registry.

Until the transition to POLST, Massachusetts is still using the MOLST form. 

If you already have a MOLST, your MOLST is still valid and will be honored even after the transition to POLST.

What is POLST?

POLST is a form that documents the treatments you would want in a serious medical emergency. It is a medical order that health care providers follow when you are so sick that you can’t communicate.

Who is POLST for? 

POLST is for people with serious illness whose health is getting worse, and who want to avoid certain treatments in an emergency. 

POLST is voluntary, never required.
