- For name change or duplicate license, you MUST return your current license with the form below.
- For address changes only, DO NOT return your current license. All addresses are subject to disclosure upon request, M.G.L. c4,s7.

Fees are non-refundable and non-transferable. Payments may be made with a check or money order. Please make checks or money orders payable to The Commonwealth of Massachusetts. DO NOT SEND CASH.
