You will be asked to submit all required documents via the eLIPSE platform, including a DOL CORI form and a photo. The CORI form must be notarized.

- Log into your eLIPSE account;
- Click the “Request Support” button on your main page or in the top right corner;
- Click “General Support Request”; and
- Complete the form and submit it.
