Pursuant to 239 CMR 4.12(5), all Pre-Need Reports are due by January 31 of each year. All information must be entered and submitted online. The Board will not accept mailed in reports.
DO NOT LEAVE ANY FIELDS BLANK. If the answer to any question is zero (0) or is not applicable (N/A), please indicate as such. Incomplete or unsigned reports will not be accepted. Every Funeral Home establishment must submit this report annually for each licensed location. Failure to submit a complete and electronically signed Pre-Need Report online by January 31, for each location, may result in Board discipline.
