Veterinary medicine declawing reporting form

Online declawing reporting form to be filled out if you perform declawing procedures.

Pursuant to Massachusetts General Laws as defined by Chapter 345 of the acts of 2024, you are required to submit the information below to the Massachusetts Board of Registration in Veterinary Medicine if you have performed the declawing procedure between April 8th, 2025 through December 31, 2025. This form must be completed by March 30, 2026.

The Board of Registration of Veterinary Medicine may request further information from you to determine if the procedure and documentation thereof is medically necessary and in compliance with this law.

Pursuant to this law, if this number is one or more, you are required to maintain records related to that patient as follows:

A veterinarian who performs a declawing procedure on a cat shall keep a record of the procedure for a period of 4 years after the last contact with the cat. 

The record shall include: 

  • The name and address of the cat’s owner.
  • The name and address of the person from whom payment is received for the procedure.
  • A description of the cat, including, but not limited to:
    • The cat’s name, breed, date of birth, sex, color, markings and current weight.
    • The date and time of the procedure.
    • The reason the procedure was performed.
    • Any diagnostic opinion, analysis or test result to support the diagnosis. 

These records shall be subject to audit by the board.

If you have not performed any procedures defined by this law, you are NOT required to complete and submit this form.
