Veterinary medicine devocalization reporting form

Online devocalization reporting form to be filled out if you perform devocalization procedures.

Pursuant to Massachusetts General Laws as defined by Chapter 272, Section 80 1/2, you are required to submit the information below to the Massachusetts Board of Registration in Veterinary Medicine if you have performed the devocalization procedure between January 1st, 2025 through December 31, 2025. This form must be completed by February 28, 2026.

The Board of Registration of Veterinary Medicine may request further information from you to determine if the procedure and documentation thereof is medically necessary and in compliance with this law.

Pursuant to this law, if this number is one or more, you are required to maintain records related to that patient as follows:

A veterinarian who performs a surgical devocalization procedure on a dog or cat shall keep a record of the procedure for a period of 4 years after the last contact with the animal. 

This record shall include: 

  • The name and address of the animal’s owner.
  • The name and address of the person from whom payment is received for the procedure.
  • A description of the animal, including its name, species, breed, date of birth, sex, color, markings and current weight.
  • The license number and municipality that issued the license for the animal.
  • The date and time of the procedure.
  • The reason the procedure was performed.
  • Any diagnostic opinion, analysis or test results to support the diagnosis.

These records shall be subject to audit by the board.

If you have not performed any procedures defined by this law, you are NOT required to complete and submit this form.
