What you need for Apply for a Drinking Water Supply Facility Operator's license
Please note: All applications (except upgrades and reciprocity) must be submitted within 6 months of passing an examination in order to be considered for licensing.
The following should be uploaded to your application upon submittal:
- A recent 2” x 2” passport-style photo.
- The ABC score report verifying you have passed the exam.
- All applicants with greater than a high school diploma, GED or equivalent must submit proof of such education. Candidates with a BS, AS or MS must submit a copy of their diploma or college transcript. All other degrees or non-degree college experience must include a copy of the transcript.
- Proof of Experience:
- All applicants seeking a certificate for “Full” status must include a copy of their job description obtained directly from their employer or a letter from their supervisor detailing their duties and responsibilities. Candidates must include verification from their employer(s) of years of service and hours worked per week. In addition, the applicant must fill out this Experience Form. You may use it as many times as needed.
- All applicants for Grade 2 or higher-level exams must submit a copy of the Certificate of Completion issued by their training organization to demonstrate that the applicant has successfully completed the required training course(s) for the grade and classification of the license being applied for. The required training course(s) include the following:
- D2, D3, or D4: Applicant must complete the Basic Distribution Training Course with a provider approved by the Board.
- T2: Applicant must complete the Basic Treatment Training Course with a provider approved by the Board.
- T3 or T4: Applicant must complete the Advanced Treatment Training Course with a provider approved by the Board.
Applicants may apply for a waiver from the training requirements if they meet criteria established by the Board. If an applicant has been granted a waiver, the applicant must submit a copy of the approved waiver.
- Criminal Offender Record Information CORI Acknowledgement form.
- Signed Code of Ethics Agreement.
- Veterans only: A copy of your DD form 214.
Fees for Apply for a Drinking Water Supply Facility Operator's license
Fees are non-refundable and non-transferable.

Note: This may not include additional fees, such as licensure fees, which may be assessed later and there is a separate convenience fee charged by DOL’s vendor for certain online payment transactions.
