What you need
To complete an application, enter all required information and submit the following documents:
- A 2x2 passport-style photo.
- A copy of applicant's High School Diploma or equivalent.
- A copy of OSHA Certificate showing class was completed within 30 days prior to applying.
- Complete Certification for Funeral Assistant Form.
- Complete notarized CORI Acknowledgement Form.
Fees
Payments may be made with a check or money order. Please make checks or money orders payable to The Commonwealth of Massachusetts. Fees are non-refundable and non-transferable.

How to apply
- Applications and payments for apprentice licenses must be submitted online through the ePlace platform.
- You will be asked to submit all required documents via the ePlace platform, including a DOL CORI form. Once submitted, you will be notified of any required next steps.
- Incomplete applications will be held open for 180 days from the date of application receipt in order to allow applicants to provide the missing information or documentation. After 180 days have elapsed, the incomplete application will be considered abandoned, and applicants will need to submit a new application and payment.
