What you need
Required documents:
- A floor plan including:
- The layout of the whole business, showing rooms, equipment, front and back doors, bathroom location, office set-up and square footage. Each room must be labeled with how it will be used.
- The owner’s name and 2 phone numbers where they can be contacted
- The name and address of the business- this must be the street address and not a P.O. Box
- The date the business is expected to open
- A copy of the electrologist’s valid license
- A copy of the blank case history card
- A copy of the business certificate
- If the business is incorporated, submit a copy of the Articles of Incorporation
- If it’s a partnership, submit a copy of the partnership agreement
- If it's a Limited Liability Company (LLC), submit a copy of the Certificate of Organization
How to apply
- When you create your MyMassGov account to access eLIPSE the first time, eLIPSE will ask you to enter information to help it locate your license and link it to your MyMassGov account.
- Please read the Linking Your License guide before creating your account to ensure it is linked correctly.
- Applications, renewals, and reinstatements must be submitted online through the eLIPSE platform.
- You will be asked to submit all required documents via the eLIPSE platform, including a DOL CORI form. Once submitted, you will be notified of any required next steps.
- Payment will also be made via the eLIPSE platform.

- Incomplete applications will be held open for 180 days from the date of application receipt in order to allow applicants to provide the missing information or documentation. After 180 days have elapsed, the incomplete application will be considered abandoned, and applicants will need to submit a new application and payment.

Next steps
Inspection
Within several weeks after the submitted application has been received and approved, an investigator will contact the owner to set up an inspection date. If the appointment is missed, the application will be denied, and a new application will need to be submitted to reapply.
