The Details
What you need
Once you have completed the entire application (all sections must be filled in/completed), the following documents should be mailed to the Board:
- The completed application form
- Payment – either check or money order made payable to the Commonwealth of Massachusetts in the amount of $216.
The following documents should be emailed:
- Required supporting work documentation as outlined in section 24 of the instructions should be scanned/saved as pdf and emailed to eric.funk@mass.gov.
- If the file size is large, it can be split it into several emails, please just number them 1 of __.

All fees are non- transferable and non-refundable.

Incomplete applications will be held open for 180 days from the date of application receipt in order to allow applicants to provide the missing information or documentation. After 180 days have elapsed, the incomplete application will be considered abandoned, and applicants will need to submit a new application and payment.
