Board of Registration of Professional Engineers and Land Surveyors reinstatement applications

Once you have completed the entire application (all sections must be filled in/completed), the following documents should be mailed to the Board:

• The completed application form
• Payment – either check or money order made payable to the Commonwealth of Massachusetts in the amount of $216.

 The following documents should be emailed:

• Required supporting work documentation as outlined in section 24 of the instructions should be scanned/saved as pdf and emailed to eric.funk@mass.gov.  
• If the file size is large, it can be split it into several emails, please just number them 1 of __.
