  • A completed certification application.
  • A letter stating what the new name of the ALR will be and the date of when the new name will be effective.
  • Copies of the residency agreement, disclosure statement, and marketing materials which reflect the new name.
  • An explanation of how residents and/or families will be notified of the change of name.
