Instructions for Filing an Appeal:

1. Send a completed adjudicatory hearing fee transmittal form and check or money order of $100.00 payable to the Commonwealth of Massachusetts to the MassDEP Lockbox at:

P.O. Box 4062

Boston, MA 02211

2. Send the following items to the address below by mail, hand delivery, e-mail or fax.

  • A copy of the adjudicatory hearing fee transmittal form,
  • A copy of the check or money order,
  • Notice of Claim, and
  • A copy of the document being appealed to:
