Instructions for Filing an Appeal:
1. Send a completed adjudicatory hearing fee transmittal form and check or money order of $100.00 payable to the Commonwealth of Massachusetts to the MassDEP Lockbox at:

Adjudicatory Hearing Fee Transmittal Form
2. Send the following items to the address below by mail, hand delivery, e-mail or fax.
- A copy of the adjudicatory hearing fee transmittal form,
- A copy of the check or money order,
- Notice of Claim, and
- A copy of the document being appealed to:
