What you need for LES 03 EA: Modification of Certification for Chemical Laboratory
Anyone (private corporation, individual, partnership or association, or other entity) who intends to conduct analytical measurements for the purposes of determining compliance with MassDEP standards where the MassDEP or the laboratory’s client requires such measurements be conducted by a certified laboratory must apply for certification from the Department.
What is needed to apply:
- Current MassDEP Laboratory Authorization Number and EEA ePlace account
- Results of Proficiency Tests (PTs) submitted by the laboratory's PT provider in accordance with the current Senator William X. Wall Experiment Station PT policy,
- A Laboratory Quality Assurance Plan,
- Standard Operating Procedures,
- Resident State Certificate and Parameter List (for laboratories located outside of the Commonwealth of Massachusetts),
- Resident State On-site Inspection Report (for laboratories located outside of the Commonwealth of Massachusetts), and
- A complete and accurate MassDEP application form for the appropriate permit category.
