What you need   for WW03: Amendment Application (for WW01, WW14/15, WW16, WW17)

This application process has changed. Do not generate a transmittal number for this application type. Please review the instructions carefully and do not generate a transmittal number for any Chapter 91 applications.

The WW03 Amendment application is to be used for a request to renew the term of a license/permit or a change to the licensed fill/structures (provided the change is not a Substantial Structural Alteration or Substantial Change in Use - see definitions at 310 CMR 9.02).

If the authorization proposed to be amended does not have a record in the ePlace system, please first “Claim” the authorization using the instructions available in the applicable document at “1.” in Next Steps.

For an amendment to a Simplified License (WW06) please use form WW12.

  3. Submit the Application and Fee Payment

    1. File online via the EEA ePlace Portal.
    2. For additional guidance on navigating the EEA ePLACE Portal, please follow this link - EEA ePLACE Portal Instruction Documents
    3. You will have the option to pay online by credit card or automated clearing house (ACH) or pay by mail. If you select pay-by-mail option, instructions on how to do so will be provided when you submit your online application.
