Notice to Quit Accompanying Form

All landlords must deliver an accompanying form when issuing a notice to quit for nonpayment of rent to residential tenants.

This is required by Section 31 of Chapter 186 of Massachusetts General Laws. 

*Please be advised, as of April 1, 2023, landlords are no longer required to submit an attestation form or copy of the notice to quit to the Executive Office of Housing and Livable Communities.*

Required Notice to Quit Accompanying Form

Landlords Must Provide an Accompanying Form with Notice to Quit: As of April 1, 2023, landlords that issue a notice to quit for nonpayment of rent to a residential tenant must also give the tenant (1) a completed accompanying form that details information on rental assistance programs, applicable trial court rules, standing orders or emergency administrative orders pertaining to actions for summary process, and relevant federal or state legal restrictions on residential evictions; and (2) documentation of any agreements between the tenant and landlord for the tenant to repay the landlord for nonpayment of rent. This is required by Section 31 of Chapter 186 of Massachusetts General Laws.
