Offering an in-house training program

Approval for any In-Service Training Program is valid for 2 years from the date of issue. In order to be approved to offer an in-house training, organizations must provide the following:

• A completed application
• A copy of your curriculum(s) and training materials for each restriction
• Curriculum must contain the minimum topics and associated hours for those topics as listed in 520 CMR 6.07(4).
• A copy of all Company Licenses to be used for the In-Service Training Program
• A list of all the names, MA Hoisting Machinery License numbers, and legible photocopies of the InService Training Program Instructors’ MA Hoisting Licenses
• A list (including the make and model) of all hoisting machinery to be used in the In-Service Training Program.

ALL of these items MUST be submitted with this application in order for your application to be processed properly. Failure to submit all required information will result in unnecessary delays.
