To request review of a Housing or Credit Application, pursuant to G.L. c. 239, § 16(j), please email: AGOApplicationReview@mass.gov. The AGO will review to determine if the housing or credit application is missing the statutorily required notice. In its sole discretion, the AGO may issue a warning letter to the indicated party. Disclosure of Your Request
Individuals seeking to report housing or credit applications that do not comply with the requirements of G.L. c. 239, § 16(j) should submit a copy of the blank housing or credit application at issue. Your request to the AGO, including any personal information you provide, is considered a public record unless an exemption to the public records law applies.
What Should I Include?
Please be sure to include all of the following information in your email:
- The date you were asked to complete the application for housing or credit.
- The following information about each party or parties that asked you to fill out the application:
- Individual’s Full Name
- Company / Business Name
- Mailing Address
- Phone Number
- Email Address
- Attach a copy or image of the application to your email
- If the application was online, please download and attach a blank copy of the application.
- If the application cannot be downloaded, please provide a screen capture of the entire application; please also provide the website address (url) for the application if it is publicly available.
- If you received a hard copy of the application, please scan or take a picture of all pages of the application and attach a copy to your email. Please ensure the resolution of the images is clear enough for us to read the entire application.
Please do not include:
- Your personal information in the application. Where possible please provide a blank copy of the application. If you do not have a blank copy of the application, please redact (or completely black out) your personal information (name, address, social security number) from the application before submitting to the AGO.
Complaints about violations of the eviction sealing law, other than failures to include the required language in a housing or credit application:
If you believe you were denied housing or credit unlawfully because of an eviction record that was sealed at the time your application was submitted, please do not use this email address.
