Standard forms and instructions

State agencies use these forms for the procurement and management of leased space.

Facility Plan, Form 1 (A-C) and Form 1D - communicates information needed to procure leased space or amend an existing lease

Agency Recommendation Form 2communicates the agency’s proposal-selection recommendation 

Transaction Approval, Form 3communicates the agency’s rent payment obligations

Occupancy Termination Form, Form 4 documents the date on which a lease terminates
