Leasing Property to the Commonwealth

The Division of Capital Asset Management and Maintenance (DCAMM), leases more than 7.5 million square feet of space from private and public landlords.

Leasing services include acquisition of new leases and management of active leases.

Standard forms and instructions

State agencies use these forms for the procurement and management of leased space.

Facility Plan Form 1 (A-C) and Facility Plan Form 1D - communicates information needed to procure leased space or amend an existing lease.  There are two options for Facility Plan Form 1 documents for agency use. 

Agency Recommendation Summary Form 2 - communicates the agency’s proposal-selection recommendation 

Transaction Approval Form 3 - communicates the agency’s rent payment obligations

Occupancy Termination Form 4 - documents the date on which a lease terminates
