Once completed, this form is sent to the Massachusetts Office of the State Treasurer, Veterans’ Bonus Division who will process and respond to your request.
Someone from the Division will contact you with a response by text, email, or phone Monday-Friday. If you have a preferred way to be contacted include that in the service requested area.
Additionally, you can request other services in the service requested area if needed and our office will try to connect you to the appropriate agency with your consent.
The consent portion of the inquiry allows us to research your information in our database or refer you to another agency if you are requesting additional services.
Required fields are First Name, Last Name, Date of Birth, Phone Number, and Service requested.
Please note: COVID-19, Armed Forces Expeditionary Medal (AFEM), Persian Gulf War, Vietnam conflict, Korean war, and World War II are one-time bonuses. The Veterans’ Bonus Division can verify if you were previously awarded the bonus.
